Central Florida Health – Discharge Timeout Reduces Medication Errors

After discovering that many of their CHF readmissions were caused by patients going home with multiple medication discrepancies, Central Florida Health reviewed their internal processes and identified opportunities at discharge.  This led them to pilot a new “Discharge Timeout” process, which reduced the average percent of patients discharged with a medication error from 30% to 5%.· Continue reading (Portal login required)
